Player Roster

Players 1–7 are required. Players 8–10 are optional. The team captain must be listed as a player.

Acknowledgments

I have read and agree to the Official Tournament Rules. The captain is responsible for communicating all rules to the full roster before Day 1.

I understand the entry fee is $1,500 flat regardless of roster size. A non-refundable $750 deposit is due within 48 hours of submitting this form. The $750 balance is due by September 12, 2026.

All players must sign the liability waiver before competing. Players under 18 require a guardian signature. By registering, players consent to being photographed and filmed for DSF promotional use.
